Portfolio Assessment Checklist

To be a candidate for licensure, all of the following items must be completed and verified by SOE portfolio reviewers/evaluators.

Major Components:

  ______ A complete table of contents displaying all portfolio components.
______ A detailed and professional autobiography describing the candidate’s life in terms of educational goals.
______ A clear educational philosophy statement.
______ A resume of the candidate’s qualifications and experiences appropriate for seeking employment in education.
______ A completed Alignment Summary documenting alignment of artifacts to the twenty-two UW Platteville Knowledge, Skill, and Disposition Statements and to the ten Wisconsin Standards for Teacher Development and Licensure.
______ The portfolio is mechanically correct (e.g., no spelling, grammar, and punctuation errors).
______ A reflective Plan for Professional Development including an assessment of how the candidate
will build on strengths and address weaknesses in professional development beyond initial licensure.

  Artifacts:
    ______ demonstrate competence in each of the ten Wisconsin State Teacher Standards.
______ demonstrate evidence of work of increasing depth, meaning, and relevance over time.
  Reflections for each artifact:
    ______ demonstrate competence in the standard(s) by describing what the student knows about the process of teaching, learning and how these concepts relate to the development of students.
